New family packet
Client Intake Form
Tells us about your child, your family, goals, and any health or school information. Required for all new clients.
Authorization to Exchange Records
Allows us to share records with your child’s school, clinic, or other providers so everyone can work together.
Transportation Authorization & Waiver
Gives permission for staff to drive your child to outings and activities.
Photo & Social Media Release
Lets you choose whether photos of your child may be used in our materials. You can change your choice at any time.
If needed
Medication Authorization
Required only if your child needs medication during sessions. Page 2 is completed by your child’s prescribing provider.
Keeping your information private
Some forms ask for health and medical details. Please return completed forms in person or by mail rather than by email or Facebook message.
